Comparative Study of Indirect Immunofluorescence and Immunoblotting for the Detection of Autoantibodies in Epidermolysis Bullosa Acquisita




Abstract
The diagnosis of epidermolysis bullosa acquisita (EBA) relies on immunopathological criteria including detection of circulating autoantibodies to type V¥±collagen. The purpose of this study was to compare the sensitivity and usefulness of
indirect
immunofluorescence(IF) using NaCl-split skin substrates with that of immunoblot assay using amplified alkaline phosphatase staining system in the detection of EBA autoantibodies. An equivalent study has been performed for pemphigus and bullous
pemphigoid in connection with the diagnostic usefulness, but not, to our knowledge, has it been carried out for EBA. Among four sera taken from patients with known EBA, the titers of IgG autoantibodies found on indirect IF were 1:80~1:60;
however,
the
titers examined by immunoblot assay were found to be 1:640~1:1280, higher than those results of indirect IF(p=0.068). The indirct iF can be performed easily and seems favorable in the aspect of cost effectiveness. An immunoblot assay with
sensitive
staining method would be warranted in cases of antibody-negative on indirect iF or of atypical EBA cases to confirm the diagnosis.
